Yes, I understand that the gal in your ER sees my partner, Dr. Johnson. But you are telling me that she is orthostatic, hypotensive, and blood is pouring out her vagina. I will NOT accept an unstable patient from a 45 minute drive away, so please call Dr. Smith at your hospital. She has GYN privileges at your hospital. And yes, I completely understand that she sees my partner. She could possible DIE in transit. Call Smith.
I needed to repeat that multiple times before he understood no means no.

I run the OB department at a free/medicaid clinic. But due to being in an underserved area, we also have a significant commercial insurance presence as well. The only time I look at insurance is when I'm done with the patient and & I need to know what codes to fill in for billing (medicaid and commercial having different billing requirements).
mom- i see plenty of patients referred from a distance because a more local doc will not see them due to payment issues, or bounced from the other on call practice in town for the same reason. sorry my question was unclear. that was my only point. clinically, for the welfare of this woman, you are, of course, spot on.
